Too little is being done to prevent violence against women and girls despite it being declared a national priority, a damning report has found. Lady Elish Angilolini, who is heading an inquiry set up following the murder of Sarah Everard , said she found an "unacceptable level of inconsistency" across police forces. She said "too many" offenders are slipping through the cracks as a result of a "critical failure" to record crime. In a heartbreaking statement to the inquiry, Sarah's mum Susan
