Woman dies after private scan spotted lung cancer but family claims no-one told her

A grandmother has died with her private healthcare provider being accused of failing to tell her a tumour had been spotted on her lungs during a scan. Margaret Jones' relatives say a "catastrophic failure” meant information was not passed on, meaning she wasn't diagnosed with lung cancer until four years later, by which time the condition was terminal.
